Worksafe has launched an investigation into the deaths of two workers killed at a Perth construction site on Wednesday.

A witness told the ABC the men were killed when a concrete slab fell on top of them as it was unloaded from a delivery truck.

Although a WA Police spokesman did not confirm a cause of death to The New Daily, he said they were called to an incident at a construction site on the corner of Goderich and Bennett streets, in East Perth, about 11.35am.

• Paid domestic violence leave ‘worth considering’
• Newborn abandoned in nativity scene manger
• One dead in hostage ‘operation’

The witness, Travis, said a group of workers had tried to pry the slab off of the men.

“A whole bunch of guys jumped on that side while a few of us lifted the other side … so it was a couple of inches off them,” he said.

St John Ambulance and the Department of Fire and Emergency Services also attended the incident.

The ABC reported the men died instantly.

In a separate workplace incident in Perth, a third man was believed to have died at an aluminium refinery in Kwinana, after falling from scaffolding on Wednesday afternoon.

Department of Mines and Petroleum State Mining Engineer Andrew Chaplyn told the ABC the scaffolder was believed to have fallen 10m while working at the Alcoa refinery.

“Inspectors from the department will be onsite this afternoon to investigate the incident,” he said.

“The department offers its sincere condolences to the worker’s family, friends and colleagues.”

A Worksafe spokeswoman told The New Daily officials had gone to the site soon after the incident and would investigate the deaths.

WA Police will prepare a report for the coroner.

The men were yet to be formally identified.
